This consent form written specifically during the COVID pandemic describes in client-friendly language all of the required issues that must be legally shared with clients regarding the provision of telehealth. It includes language to clarify the assessment of suitability for the service, risks, licensing issues, emergency planning, the rationale for my needing the client’s location, emergency procedures, use of HIPAA-compliant technology, how I manage tech failures, how to manage the unique clinical and boundary issues that come up when the client’s home is now “my” or “our” office, record-keeping, reporting issues, and how to file a complaint against me.
